What your client gets
This page is for discharge planners, occupational therapists, home care agencies, realtors and elder law attorneys. A family you send us gets the Livability Assessment, a walk through the whole house and a written report that ranks every problem by danger, with its fix and the order to do the work.
Nothing happens after that unless the family asks. If they want the work done, each project gets a written scope and a fixed price first, and our own crews build it.
The report you can put in a file
With your client's permission, we send you a copy. It shows what we found room by room, what is being fixed and in what order. For a discharge plan, a care plan or a listing, that's a written record that someone looked at the house and what it needs.
Where you fit
If you plan discharges or work on a rehab team, you know the patient who can't go home to that bathroom or those front steps. Send the family with the discharge date and the ramp, bath and rail work gets planned against it.
Occupational therapists assess the person, and we build what the house needs. If you've written a home evaluation, we work from it.
Home care aides see the loose rug and the high tub wall every shift. A safer house protects your client and your staff at once.
Realtors meet the senior who has to fix the house before selling it, and the family buying near a parent whose new place needs changes. Elder law and estate attorneys see plans that only work if the client can stay home. Senior centers, churches and community groups hear members ask who to call about the house.
How to refer
Have the family book the assessment on this site and mention your name, or get in touch to set up a referral relationship. We don't pay for referrals and we don't sell leads. The client is the only one we answer to.
